The Reablement Team - part of the Care at Home service – deliver amazing work, helping people to live independently at home after illness and injury.
In the video – shown as part of the Leadership Conference - Vivian Mills, an Occupational Therapist, explains the role she, and others in the team, play in setting goals and targets, and crucially how they help build the confidence and independence of service users.
Also speaking is Abby McCulloch, a care home manager for the Irvine reablement team. Abby manages a team of 24 carers and assesses what care packages someone needs when they leave hospital.
They and their teams play such an important role in the rehabilitation of service users and are a real asset to the Health and Social Care Partnership.
